Here's what insurance asks for. If something's missing, that's okay – we'll walk you through it.
- Insurance cardA photo of the front and back is fine.
- Physician referralA note or prescription for ABA (applied behavior analysis) from your child's doctor.
- DiagnosisYour child's autism diagnosis. Some plans ask for specific testing – we'll let you know if yours does.
- Intake & consent formsA short packet we send you to sign.
